Laser Forms and e-Forms

FreeForms simplifies the task of printing forms complete with data which is automatically filled in from the information in the database. Printing the paperwork for an admission, discharge, readmission or transfer is simple and quick.

Eliminate the need to store the blank forms, save space. Just print the completed forms directly on your laser printer.

The program accesses the ADL's Census/Registration and Waiting list databases and automatically inserts resident information onto form templates. To modify the forms, changes are made to the templates and replaced in the form template library.

e-Forms Repository

The enterprise version of FreeForm provides for the user to save the form and data into a Repository. The Repository maintains a comprehensive archive of all documents required the patient. Completed forms in the Repository can be viewed through the ADL program or using a browser. Now all the forms can be instantly referenced. Assembling or accessing patient paper has never been Easier.

Create Your Own Forms

Almost any existing form can be convert into a template that can be used by this module. All you need to create the templates is any program that can create Adobe Acrobat PDF Files. You can even scan a form and save it as a PDF file. Many programs have a built in functions to save documents as PDF files.

Once the form is saved as an Adobe PDF document and then enhanced with Acrobat Professional to insert the desired fields of information. FreeForm can auto-fill and read data typed into these document types.

Use this software to to automate the paperwork prepared by the Admissions Office. All the paper forms needed by that facility can be defined to FreeForm. Enter the data once into system and the information is used to auto-fill your forms. Entire set of admission or discharge forms can be printed on the click of a button.
