MDS Section GG and New GG Data Collection and eForm

 
ADL introduces online GG collection forms and auto-fill function to support MDS Section GG. This are difficult enough for the direct care staff to document accurately and now there is a new set of definitions for the same or similar ADLs that also needed to be accurately coded. In addition, this GG Coding has different timelines for completion from the original MDS fields.

 
The GG data needs to be captured:

• At Start of SNF PPS stay, with an assessment period of days 1-3
• At End of SNF PPS stay upon discharge

 
Section GG the “Self Care” item does not have the separate coding for self-performance and support as are used to in Section G: rather the admission performance and discharge goal coding is combined with amount of assistance needed.

 
Accessing the GG Collection Tool needs to be integrated with the Admission and Discharge (ADT) Workflows.

ADL Interfaces with Real Time Medical Systems

Announcing the availability of Interface with Real Time Medical Systems. The New Interface continuously transmits EMR data to TranSights for processing and provides a direct link to their console.

TranSights™ Clinical provides care staff with detailed clinical alerts and intervention recommendations to ensure the appropriate care is administered, improving quality of care and reducing hospital readmissions.

Proactively manage quality data and outcomes with “Live” Quality Measures. TranSights uses real-time clinical documentation to generate daily QMs. These calculated care measures allow staff the opportunity to place corrective measures in place to reduce these measures in real-time. CMS Infection Control Pilot Project 2015-12-23

Project Overview

The Centers for Medicare & Medicaid Services (CMS) has begun a three year pilot project to improve assessment of infection control and prevention regulations in nursing homes, hospitals, and during transitions of care.

Survey details

All surveys during the pilot will be educational surveys (no citations will be issued) and will be conducted by a national contractor. New surveyor tools and processes will be developed and tested, focusing on existing regulations as well as recommended practices (such as those for antibiotic stewardship and transitions of care). Ten pilot surveys to be conducted in Fiscal Year (FY) 2016 will occur in nursing homes. Surveys in FY17 and FY18 will be conducted in nursing homes and hospitals.

Project Outcomes

New surveyor infection control tools and survey processes that can be used to optimize assessment of new infection control regulations.

New MDS 3.0 CAT v1.02.0 Specifications

CMS has released a new version (v1.02.0) of the Care Area Trigger (CAT) specifications for OBRA comprehensive assessments with an assessment reference date on or after 10/01/2016. Please note the changes to CATs 1 and 2, which were necessary due to the removal of the C1300 items in v1.14.0 of the MDS 3.0 item sets. Download New CMS Documents for the 2016 Release of MDS

CMS has released 3 new documents related to the Oct. 1, 2016, release of MDS:

  • The MDS final 2016 Part A Discharge (End of Stay) (NPE/SPE) item set
  • The final MDS 3.0 Sections A and GG
  • The final SNF Quality Reporting Program specifications for the quality measures adopted through the FY 2016 final rule
